ABSTRACT

Colorectal Cancer (CRC) is the third most frequent neoplasia worldwide. Despite the significant advances in surgical techniques and the development of new targeted antineoplastic therapies for this type of tumor, primary prevention and early diagnosis of malignant precursor lesions will continue to be the best strategies to reduce their incidence, morbidity, and mortality. Technologies for CRC screening can be classified into two groups, those of an invasive nature, such as colonoscopy and all its different modalities of use, and those of a non-invasive nature, such as laboratory tests and imaging. This review, will focus exclusively on non-invasive screening tests, excluding imaging. Specifically, it will address those that use depositions as a sample. This review will approach the latest international recommendations, regarding the age at which they should be used, their technical-biological bases, the two main types currently used (biochemical and immunological), and we will put into perspective their advantages and their possible disadvantages. Towards the end of this article, the most recent biotechnological developments in relation to molecular tests based on the study of blood samples, will be discussed. Although these tests are not yet in routine clinical use given their high costs, they are promising for the early detection of CRC.


El cáncer colorrectal (CCR) es la tercera neoplasia más común en todo el mundo. A pesar de los avances significativos en las técnicas quirúrgicas y en el desarrollo de nuevas terapias antineoplásicas para este tipo de tumor, la prevención primaria y el diagnóstico precoz de lesiones precursoras malignas siguen siendo las mejores estrategias para reducir la incidencia, morbilidad y mortalidad asociadas al CCR. Existen dos tipos de tecnologías para el tamizaje del CCR: las invasivas, como la colonoscopia, y las no invasivas, como los ensayos de laboratorio y la imagenología. Esta revisión, se centrará exclusivamente en las pruebas de tamizaje no invasivas que utilizan muestras de deposiciones, excluyendo las imágenes. Se abordarán las últimas recomendaciones internacionales sobre el momento etario en que se deben utilizar, sus bases técnico-biológicas, los dos principales tipos utilizados en la actualidad (bioquímico e inmunológico) y pondremos en perspectiva sus ventajas y posibles desventajas. Al final de esta revisión, se discutirá brevemente los últimos avances biotecnológicos relacionados con pruebas moleculares basadas en el estudio de muestras sanguíneas. Aunque estas pruebas aún no son de uso clínico habitual debido a sus altos costos, representan una prometedora innovación para la detección temprana del CCR.

Resumen: El ámbito laboral tiene un gran potencial para difundir información e implementar actividades de promoción de la salud, como la prevención y detección temprana del cáncer. Debido al desafío de implementar intervenciones sanitarias en este ámbito, se necesitan investigaciones formativas para adecuar el diseño y la implementación de las estrategias para que sean exitosas. Con el objetivo de informar el diseño e implementación de la intervención, para mejorar la tasa de adherencia al tamizaje con el test de sangre oculta en materia fecal, en un ámbito laboral en Argentina, se realizó una investigación formativa para identificar potenciales barreras y facilitadores. La investigación formativa siguió un diseño metodológico cualitativo. Se realizaron entrevistas con 10 responsables de áreas clave de la institución en que se desarrolló el estudio y 8 grupos focales con trabajadores mayores de 50 años. Se identificaron desafíos para la implementación de la intervención en el nivel institucional, como la dispersión geográfica de los trabajadores y la complejidad del organigrama de la institución, y en el nivel de la población objetivo, como el bajo conocimiento del cáncer colorrectal y de los test diagnósticos, así como la baja percepción de riesgo. Entre los facilitadores se destacaron la aceptabilidad de la intervención, además de la disponibilidad de recursos humanos y materiales para llevar adelante la intervención, como canales de comunicación institucional para la diseminación de información, así como la logística para la distribución de kits diagnósticos. La investigación formativa permitió relevar recursos e identificar potenciales barreras que informaron el diseño y la implementación de la intervención.


Abstract: The workplace has great potential to disseminate information and implement health promotion activities such as cancer prevention and early detection. Due to the challenges of deploying health interventions in this setting, formative studies are needed to adjust the design and implementation of successful strategies. To inform the intervention's design and implementation and improve the adherence rate to screening with fecal occult blood test in a workplace in Argentina, a formative study was conducted to identify potential barriers and facilitators. The formative study adopted a qualitative methodological design. Interviews were held with 10 individuals in charge of key areas in the institution where the main study was conducted, besides 8 focus groups with workers over 50 years of age. Challenges were identified at the institutional level for the intervention's implementation, such as workers' geographic dispersion and the complexity of the institution's flowchart, as well as at the population level, such as low knowledge about colorectal cancer and diagnostic tests and low risk perception. The facilitators featured the intervention's acceptability and the availability of human and material resources to proceed with the intervention, such as institutional communications channels for disseminating the information and logistics for distribution of diagnostic kits. The formative study allowed identifying resources and potential barriers that informed the intervention's design and implementation.


Resumo: O local de trabalho possui grande potencial para divulgar informação e implementar atividades de promoção da saúde, como a prevenção e detecção precoce do câncer. Frente ao desafio de implementar ações sanitárias naquele ambiente, são necessárias pesquisas formativas para adequar a estruturação e a implementação das estratégias para que sejam bem-sucedidas. Com o objetivo de subsidiar a concepção e a implementação da ação, visando melhorar a taxa de aderência ao screening com o teste de sangue oculto nas fezes no ambiente de trabalho na Argentina, foi desenvolvida uma pesquisa formativa para identificar potenciais barreiras e facilitadores. A pesquisa formativa seguiu um formato metodológico qualitativo. Foram realizadas entrevistas com 10 gerentes de áreas chaves da entidade onde foi desenvolvido o estudo e 8 grupos focais, com trabalhadores acima de 50 anos de idade. Foram identificados alguns desafios para a implementação de uma ação em nível institucional, como a dispersão geográfica dos trabalhadores e a complexidade do organograma da entidade, e em termos de população objetiva, como o baixo conhecimento a respeito do câncer colorretal e dos testes diagnósticos, além da baixa percepção de risco. Entre os facilitadores, destacaram-se a aceitabilidade da ação e a disponibilidade de recursos humanos e materiais para levar a cabo a ação, como canais de comunicação institucional, para a divulgação da informação e a logística, para a distribuição dos kits diagnósticos. A pesquisa formativa revelou recursos e identificou potenciais barreiras, ajudando na estruturação e implementação da ação.

ABSTRACT

Abstract Cancer is a leading cause of death worldwide and is expected to remain a public health concern for years to come. Within Latin America, Uruguay has the highest colorectal cancer rates. Heeding past calls to action, in this article we provide a critical assessment of colorectal cancer needs and opportunities in Uruguay with a focus on developing a roadmap for future action. First, we provide an overview of risk factors, screening procedures and guidelines, and screening rates. Next, we provide an overview of psychosocial factors that influence colorectal cancer screening, with the goal of providing guidance for future behavioral health promotion initiatives in Uruguay. In this effort, we present four conceptual models that may be used for interventions: the ecological systems theory, informed decision-making, the health beliefs model, and the health literacy model. Subsequently, we propose using an integrated model based on the ecological systems theory and health literacy model to develop national, local, and community-based interventions to increase screening rates and lower the colorectal cancer burden in Uruguay. We close the paper with a summary and implications section, including recommendations for future research programs focused on the assessment of factors that influence screening.

ABSTRACT

Colorectal cancer is a malignancy with high mortality. Huangqin Tea(HQT) can exert potential preventive and therapeutic effects on colorectal cancer. Flavonoids are the main compounds in HQT, but the pharmacodynamic material basis and mechanism are unclear. Network pharmacology and molecular docking were used to predict and analyze the targets and signaling pathways of HQT in the prevention and treatment of colorectal cancer. The active components of flavonoids in HQT were searched and screened out by literature review and FAFDrugs4. The related targets of active components were predicted by SwissTargetPrediction, STITCH, and TCMSP. Colorectal cancer-related genes were collected from OMIM, TTD, and GeneCards. The common targets were obtained as the potential targets of HQT in the prevention and treatment of colorectal cancer. Metascape was used for GO function enrichment and KEGG pathway enrichment analyses. Cytoscape was used to construct the protein-protein interaction(PPI) network and "component-target-disease-pathway" network to obtained and analyze core targets and key components. AutoDock Vina was used for molecular docking verification of key components and core targets. The results showed that apigenin, luteolin, wogonin, and baicalein were presumedly the key active components in the prevention and treatment of colorectal cancer, and core targets included TP53, AKT1, VEGFA, PIK3 CA, and SRC. The key KEGG signaling pathways mainly involved PI3 K-AKT, AGE-RAGE, p53, NF-κB, Wnt, Hippo, and calcium signaling pathways. Further molecular docking results showed that four key components showed strong hydrogen bonding ability with the five core targets. This study preliminarily reveals the pharmacodynamic material basis and potential mechanism of HQT in the prevention and treatment of colorectal cancer and provides a theoretical and scientific basis for the application of HQT.

ABSTRACT

Introducción: El cáncer colorrectal (CCR) es la segunda causa de muerte por cáncer en Argentina. Debido a su alta prevalencia es fundamental normatizar un programa de pesquisa para la prevención y detección temprana. La precisión del test de sangre oculta en materia fecal inmunológico (SOMFi) para pesquisa de CCR en población de riesgo promedio ha demostrado ser adecuada según la bibliografía internacional, no habiendo, sin embargo, información a nivel local. El objetivo es evaluar en nuestro medio la precisión diagnóstica del test de SOMFi en una única ronda para la pesquisa de CCR en pacientes de riesgo promedio. Diseño: Prospectivo de precisión diagnóstica. Material y Método: Se incluyeron pacientes con riesgo promedio que consultaron para realizar una videocolonoscopía (VCC) por pesquisa de CCR en el Hospital Alemán de Buenos Aires, entre el 1 de junio del 2015 y 31 diciembre de 2017. Se excluyeron todos los pacientes con riesgo incrementado para CCR. Todos los pacientes realizaron el test de SOMFi y posteriormente la VCC. Los endoscopistas estaban ciegos para el resultado del test al momento de realizar la VCC. Se evaluó la precisión diagnóstica del test SOMFi para detectar lesiones neoplásicas avanzadas (LNA) calculando la sensibilidad (S), especificidad (E), valor predictivo positivo (VVP) y negativo (VVN), coeficiente de probabilidad positivo (CP+) y negativo (CP-). Se evaluó también la precisión para la detección de adenomas de bajo riesgo, pólipos aserrados y CCR. Resultados: Se incluyeron un total de 300 pacientes; 273 (91%) entregaron la muestra de materia fecal para realizar el test de SOMFi y completaron la VCC. La edad media de los pacientes fue de 56.9 (40-85) años y 54% fueron hombres. Del total de pacientes que realizaron ambos estudios (273), 53 pacientes (19%) presentaron al menos un adenoma de bajo riesgo, en 18 pacientes (6,59%) observamos al menos un adenoma aserrado sésil y en 21 pacientes (7,7%) al menos una lesión neoplásica avanzada (LNA). Solo 4 pacientes (1.5%) presentaron CCR. En cuanto a la precisión diagnóstica del test de SOMFi en una única ronda para detectar LNA observamos una S de 30%, E de 84%, VPP de 13% y un VPN de 94%. Para adenomas de bajo riesgo observamos una S de 13%, E de 84%, VPP de 17%, VPN de 79%. Para adenomas aserrados sésiles observamos una S de 16.7%, E de 87%, VPP de 11% y de VPN 91%. La precisión para el CCR fue la siguiente, S de 75%, E de 83%, VPP 6%, VPN 99%. No se observaron complicaciones post procedimientos. Conclusiones: La precisión diagnóstica del test de SOMFi en nuestro medio es comparable a los resultados internacionales. Sin embargo, la baja precisión observada en una única ronda realza la necesidad de realizarlo de forma anual o bianual para poder optimizar su precisión y lograr programas de pesquisa efectivos.(AU)


Background: Colorectal cancer (CRC) is the second leading cause of cancer death in Argentina. Due to its high prevalence, it is essential to standardize a screening program for prevention and early detection. According to international literature, the accuracy of the immunochemical fecal occult blood test (FIT) for CRC screening in an average-risk population has proven to be adequate, but there is no information at the local level. Objective: To evaluate the diagnostic accuracy of the FIT test in a single round for CRC screening in average-risk patients in our setting. Design: Diagnostic accuracy prospective study. Material and Methods: Average-risk patients who consulted for a CRC screening video colonoscopy (VCC) at the Hospital Alemán of Buenos Aires, between June 1, 2015 and December 31, 2017 were included. All patients with increased risk for CRC were excluded. All patients performed FIT and subsequently VCC. The endoscopists were blind to FIT result at the time of VCC. The diagnostic accuracy of FIT to detect advanced neoplastic lesions (ANL) was evaluated by calculating sensitivity (S), specificity (Sp), positive predictive value (PPV), negative (NPV), positive likelihood ratio (LR+) and negative likelihood ratio (LR-). The accuracy for the detection of low-risk adenomas, serrated polyps and CRC was also evaluated. Results: A total of 300 patients were included; 273 (91%) submitted the stool sample to perform FIT and completed VCC. The mean age of patients was 56.9 (40-85) years and 54% were men. Of the total number of patients who carried out both studies (273), 53 (19%) patients had at least one low-risk adenoma, 18 (6.59%) patients had at least one sessile serrated adenoma and 21 (7.7%) patients had at least one ANL. Only 4 (1.5%) patients presented CRC. The diagnostic accuracy of FIT in a single round to detect ANL was: S 30%, Sp 84%, PPV 13%, NPV 94%; for low-risk adenomas: S 13%, Sp 84%, PPV 17%, NPV 79%; for sessile serrated adenomas: S 16.7%, Sp 87%, PPV 11%, NPV 91% and for CRC: S 75%, Sp 83%, PPV 6%, NPV 99%. No post-procedure complications were observed. Conclusions: The diagnostic accuracy of FIT in our setting is comparable to international results. However, the low precision observed in a single round highlights the need to do it annually or biannually in order to optimize its accuracy and achieve effective screening programs. (AU)

Abstract: Objective: The Healthy Aging Partnerships in Preven tion Initiative (HAPPI) aims to increase the use of clinical preventive services (CPS) among underserved Latinos and African Americans in South Los Angeles who are 50+ years old. Materials and methods: HAPPI uses an evidence-based model, SPARC, to leverage existing resources and link community resources. HAPPI's multi-sectoral partnerships include local non-governmental organizations (NGOs), community health centers (CHCs), aging and public health agencies serving the City and County of Los Angeles, and a university. Activities include CHC capacity assessment and training, and community capacity-building that included a small grants program. Results: We engaged five CHCs in quality improvement activities and eight NGOs in networking and programming to increase awareness and receipt of CPS. We discuss barriers and facilitators including the success of trainings conducted with CHC providers and NGO re presentatives. Conclusion: Multi-sectoral collaborations hold promise for increasing awareness and use of CPS in underserved communities.


Resumen: Objetivo: HAPPI se propone aumentar el uso de servicios clínicos preventivos (SCP) en personas mayores de 50 años en Los Ángeles. Material y métodos: HAPPI es una colaboración intersectorial e incluye organizaciones no gu bernamentales (ONG) locales, centros de salud comunitarios (CSC), centros de servicios para personas mayores, agencias de salud pública que dan servicio a la ciudad y al condado de Los Ángeles, y una universidad pública para movilizar recur sos comunitarios y promover lazos entre las asociaciones. Sus actividades incluyen asesorar y aumentar la capacitación de CSC y la comunidad, además de un programa de becas. Resultados: Se colaboró con cinco CSC para la mejora de calidad y con ocho ONG para abrir conciencia de los SCP. Se presentaron barreras y facilitadores incluyendo el éxito de las enseñanzas con proveedores de CSC y representantes de las ONG. Conclusión: Las colaboraciones multi-sectoriales son prometedoras para amplificar conciencia del uso de SCP en personas mayores.

Introduction: Colorectal cancer (CRC) is a malignant neoplasm with major impact on health today. There is, however, an efficient method for prevention and screening, which varies in different protocols according to each institution or country. The objective is to evaluate the mortality rate and the economic cost of CRC in Brazil during the first 16 years of the 21st century. Method: A retrospective, temporal aggregation study was conducted with an exploratory, documentary quantitative approach on CRC mortality from 2000 to 2016, based on the Mortality Information System database provided by the Brazilian Ministry of Health. Results: In the study period, 218,000 deaths due to CRC were recorded. The CRC mortality rate was 6.2 (95% confidence interval, 5.59-6.81) per 100,000 population, with no significant difference between men and women. Of the 17 age subgroups analyzed, eight had a significant increase from 2000 to 2016, including all subgroups aged over 50 years. Conclusion: There was an increase in mortality due to CRC in the study period.

Abstract Purpose: To evaluate red propolis, gum arabic and L-lysine activity on colorectal preneoplastic lesions induced by azoxymethane (AOM). Methods: The study featured 4 control groups (I-IV) and 4 experimental groups (V-VIII), totaling 48 rats. Once a week for 2 weeks, animals on control groups received saline, while animals in experimental groups received azoxymethane (15 mg/kg i.p.). The follow up along 16 weeks included daily oral gavage to administer water (I and V), L-lysine (150 mg/kg)(II and VI), própolis (100mg/5ml/kg)(III and VII), or gum arabic (5ml/kg)(IV and VIII). Was performed surgery on the animals in the end of this time in order to collect blood for biological assays (TBARS, GSH), followed by their sacrifice to tissue extract. Results: Oxidative stress (TBARS) and the number of aberrant crypt foci (ACF) in distal colon were lower using própolis (p<0.01 for both parameters). Gum arabic reduced preneoplastic lesions (ACF ≤ 4 crypts) on distal colon and on the entire colon (p<0.05). Conclusions: Red propolis reduced AOM-induced oxidative stress (TBARS) and total number of ACF in the distal colon. L-lysine neither protected against nor enhanced AOM-induced ACF. Gum arabic reduced the number of ACF.

ABSTRACT BACKGROUND: Polypectomy of colorectal polyps is the mainstay of colorectal cancer prevention. Identification of the best polypectomy technique is imperative. OBJECTIVE: This review aims at comparing efficacy of nine different resection methods for small colorectal polyps (<10 mm). METHODS: We searched and selected only randomized controlled trials. Primary outcome was complete resection rates of small polyps by histological eradication. Secondary outcomes were: adverse events, retrieval tissue failures rates and duration of procedure. RESULTS: Eighteen trials including 3215 patients and 5223 polyps were analysed. Overall, cold polypectomy had a significantly shorter time of procedure than hot polypectomy (RD -5.92, 95%CI -9.90 to -1.94, P<0.05), with no statistical difference on complete histological eradication (RD 0.08, 95%CI -0.03 to 0.19, P>0.05). Regarding cold polypectomy techniques, cold snare was found superior to cold forceps on complete and en-bloc resection rates and less time consuming. When comparing endoscopic mucosal resection (EMR) with hot-snare and cold-snare, the latter showed no-inferiority on histological eradication, adverse events or retrieval tissue failure rates. CONCLUSION: Cold polypectomy is the best technique for resection of small colorectal polyps. Among cold methods, dedicated cold snare was found superior on histological eradication. Cold snare endoscopic mucosal resection might be considered an option for polyps from 5 to 9 mm.


RESUMO CONTEXTO: A polipectomia de pólipos colorretais é a base da prevenção do câncer colorretal. A identificação da melhor técnica de polipectomia é imperativa. OBJETIVO: Esta revisão tem como objetivo comparar a eficácia de nove diferentes métodos de ressecção para pólipos colorretais pequenos (<10 mm). MÉTODOS: Pesquisamos e selecionamos apenas ensaios clínicos randomizados. O desfecho primário foi taxas de ressecção completa de pólipos pequenos por confirmação histológica. Os desfechos secundários foram: eventos adversos, taxas de falha de recuperação do espécime e duração do procedimento. RESULTADOS: Dezoito estudos, incluindo 3215 pacientes e 5223 pólipos foram analisados. No geral, a polipectomia a frio teve um tempo de procedimento significativamente menor do que a polipectomia a quente (RD -5,92; IC 95% -9,90 a -1,94; P<0,05), sem diferença estatística na erradicação histológica (RD 0,08; IC 95% -0,03 a 0,19; P>0,05). Em relação às técnicas de polipectomia a frio, a alça fria foi considerada superior ao uso de pinça fria nas taxas de ressecção completa e em bloco, além de um menor tempo de procedimento. Ao comparar a ressecção endoscópica da mucosa utilizando alça quente ou alça fria, esta última mostrou não-inferioridade na erradicação histológica, eventos adversos ou taxas de falha do tecido de recuperação. CONCLUSÃO: A polipectomia a frio mostrou ser a melhor técnica para ressecção de pequenos pólipos colorretais. Entre os métodos frios, a alça fria dedicada foi considerada superior na erradicação histológica. ressecção endoscópica da mucosa com alça fria pode ser considerado uma opção para pólipos de 5 a 9 mm.

ABSTRACT BACKGROUND: Colorectal cancer is one of the major causes of death worldwide. Many studies have been done on the biology of its formation as well as its treatment in recent years. One of the factors involved in the formation or treatment of this malignancy can be attributed to the microbial flora in the intestine. OBJECTIVE: This study investigate the potential preventive effect of Lactobacillus acidophilus and Lactobacillus plantarum in patients with polyps or colorectal cancer (CRC). METHODS: A total of 77 samples were selected in the form of three groups including individuals suffering from CRC, polyps and healthy subjects. Genomic DNA of fecal specimens and standard strains were extracted and amplified employing primers targeting of the 16S rRNA gene for initial detection. Absolute Real Time PCR quantification was used to determine the copy of the bacterial expression per gram of feces. RESULTS: No significant difference were observed between age and gender in the mentioned groups (P=0.06). The average copy number of Lactobacillus acidophilus shows Significant difference between the healthy group and those with polyps (P<0.0001), the healthy group and those with colorectal cancer (P<0.0001), as well as those with polyps and the colorectal cancer patients (P<0.0001). CONCLUSION: These results may indicate that taking Lactobacillus acidophilus in people with a family history of CRC and people with polyps may be a way of preventing, treating or reducing the severity of CRC.


RESUMO CONTEXTO: O câncer colorretal é uma das principais causas de morte em todo o mundo. Muitos estudos têm sido feitos sobre a biologia de sua formação, bem como o seu tratamento nos últimos anos. Um dos fatores envolvidos na formação ou no tratamento desta malignidade pode ser atribuído à flora microbiana no intestino. OBJETIVO: Este estudo investigou o potencial efeito preventivo de Lactobacillus acidophilus e Lactobacillus plantarum em pacientes com pólipos ou câncer colorretal (CCR). MÉTODOS: Um total de 77 amostras foram selecionadas e três grupos foram formados, a saber, indivíduos portadores de CCR, pólipos e indivíduos saudáveis. O DNA genomico de espécimes fecais e de amostras padrão foi extraído e amplificado empregando primers que focalizaram o gene do rRNA 16S para a deteção inicial. A quantificação do PCR em tempo real absoluto foi utilizada para determinar a cópia da expressão bacteriana por grama de fezes. RESULTADOS: Não foram observadas diferenças significativas entre idade e sexo nos grupos citados (P=0,06). O número médio de cópias de Lactobacillus acidophilus mostra diferença significativa entre o grupo saudável e aqueles com pólipos (P<0,0001), o grupo saudável e aqueles com câncer colorretal (P<0,0001), bem como aqueles com pólipos e câncer colorretal pacientes (P<0,0001). CONCLUSÃO: Estes resultados podem indicar que a ingestão de Lactobacillus acidophilus em pessoas com antecedentes familiares de CCR e pessoas com pólipos pode ser uma forma de prevenir, tratar ou reduzir a gravidade da CCR.

Introducción: la hemorragia digestiva se define como toda pérdida de sangre procedente de cualquier sitio del tubo digestivo, desde la boca hasta el ano, incluyendo las hemorragias digestivas originadas en órganos que drenan en la luz intestinal. Objetivo: realizar el diagnóstico y seguimiento de pacientes con sangre oculta positiva en heces fecales. Materiales y métodos: se realizó un estudio epidemiológico descriptivo en el municipio de Unión de Reyes, provincia de Matanzas, entre los años 2014-2015. El universo y muestra estuvo constituida por 130 pacientes a los cuales el test de sangre oculta en heces fecales fue positivo, teniendo entre las variables características sociodemográficas de los pacientes, diagnóstico y seguimiento. Resultados: dentro de los resultados obtenidos las edades comprendidas entre 50-69 años fueron las más frecuentes, así como el sexo femenino donde la positividad del test fue mayor al igual que en negros y mestizos. Los medicamentos que más incidencia tuvieron fueron los antiinflamatorios no esteroideos y la aspirina. El test le fue indicado a 115 pacientes por atención primaria siendo 76 por pesquisa. A 80 pacientes no se les comunicó con que intensión se indicaba el examen. Del total de pacientes 43 tuvieron enfermedades relacionadas con el tracto digestivo alto y 44 del bajo. Conclusiones: uno de cada cinco pacientes presentó alguna tumoración del tracto digestivo. Se consideró que el trabajo de los equipos de salud fue deficiente en el diagnóstico y seguimiento de los pacientes (AU).


Introduction: digestive hemorrhage is defined as any blood loss that stems from any site of the alimentary canal, from the mouth to the anus, including digestive hemorrhages occurring in organs draining in the intestinal lumen. Objective: to diagnose and follow-up patients with positive hidden blood in fecal feces. Materials and methods: a descriptive epidemiologic study was carried out in the municipality of Unión de Reyes, province of Matanzas, in the period 2014-2015. The universe and sample were formed by 130 patients whose test of hidden blood in fecal feces was positive, taking as variables the socio-demographic characteristics of the patients, their diagnosis and follow-up. Results: some of the obtained results were the predominance of ages between 50 and 69 years, and also the female sex with higher test positivity like in black and mestizo patients. The medications having more incidence were the non-steroidal antiinflammatory ones and aspirin. The test was indicated to 115 patient in the primary health care, being 76 in the screening. 80 patients were no informed of the intentionality when the test was indicated. From the total of the patients, 43 had diseases related with the upper digestive tract, and 44 with the low one. Conclusions: one form each five patients showed any tumor of vthe digestive tract. Introducción: El riesgo de desarrollar neoplasias colónicas en pacientes acromegálicos y su relación directa con los niveles elevados de GH/IGF-1 no están bien establecidos y continúan siendo motivo de controversia en la literatura mundial. El objetivo de este trabajo fue evaluar el riesgo de desarrollar lesiones neoplásicas avanzadas (LNA) (adenomas mayores a 1 cm, componente velloso mayor del 75% y/o displasia de alto grado), en pacientes con acromegalia, comparado con un grupo control. Materiales y métodos: Estudio multicéntrico caso-control retrospectivo. Ciento treinta y siete pacientes con acromegalia que realizaron videocolonoscopia (VCC) fueron incluidos inicialmente, aunque solo 69 cumplieron criterios de inclusión. Sesenta y dos controles fueron obtenidos: por cada caso (paciente con acromegalia) 2 «controles¼ fueron seleccionados aleatorizadamente e igualados por edad y sexo. El riesgo se expresó en odds ratio (OR) y su correspondiente intervalo de confianza (IC) del 95%. La significación estadística fue considerada una p < 0,05. Resultados: De los 69 pacientes con VCC completa y datos adecuados para su análisis, 28 presentaron VCC positiva con hallazgos de pólipos (40%) y 41 VCC negativa o normal (60%). Dentro del grupo con VCC positiva, 14 presentaron LNA (20%) y solo un paciente presentó diagnóstico de cáncer colorrectal. Para el análisis caso-control se incluyó a 31 pacientes frente al grupo control (n = 62) que cumplieron con los criterios de inclusión. La presencia de pólipos colónicos, adenomas y LNA en los pacientes con acromegalia fue de 19/31 (61,9%), 14/31 (45,16%) y 10/31 (32,25%), y en el grupo control de 18/62 (29,03%), 11/62 (17,74%) y 4/62 (6,45%), respectivamente. El riesgo de adenomas y LNA fue mayor en el grupo de acromegalia en comparación con el grupo control, siendo ambos resultados estadísticamente significativos: adenomas OR 2,54 (IC 1,22-5,25) p = 0,005, LNA OR: 7,3 (2,4-25), p = 0,00. Conclusión: La acromegalia se asocia a un mayor riesgo de lesiones colónicas preneoplásicas. Este hallazgo justifica el cribado con VCC al diagnóstico en pacientes con acromegalia.


Background: The risk of developing cancerous lesions in the colon of acromegaly patients and their direct relationship with elevated growth hormone (GH) and insulin-like growth factor-1 (IGF-1) levels is not well established, and is still controversial in the international literature. The objective of this study was to evaluate the risk of developing advanced neoplastic lesions (ALN: greater than 1 cm adenomas, villous component greater than 75% and/or high grade dysplasia) in patients with acromegaly compared to a control group. Materials and methods: A multicentre, retrospective case-control study was conducted initially on 137 patients with acromegaly (cases) who underwent videocolonoscopy (VCC), although only 69 met inclusion criteria. Sixty-two controls were obtained, and for each case two "controls" were randomly selected and matched by age and gender. Risk was expressed as odds ratio (OR) and its corresponding 95% con"dence interval (CI). P values < .05 were considered statistical significantly. Results: Of the 69 acromegaly patients with a completed VCC and adequate data for their analysis, 28 had a positive VCC with findings of polyps (40%), and 41 VCC negative with no lesions (60%). Within the group with positive VCC, 14 were ALN (20%) and one a colorectal cancer. In the case-control analysis, 31 cases were to be analysed against the control group (n = 62). The presence of colonic polyps, adenomas, and ALN in patients with acromegaly was 19/31 (61.9%), 14/31 (45.16%), and 10/31 (32.25%), respectively, and in the control group, it was 18/62 (29.03%), 11/62 (17.74%), and 4/62 (6.45%), respectively. The risk of adenomas and ALN was higher in the acromegaly group compared to the control group: adenomas OR: 2.54 (95% CI 1.22-5.25) P=.005, ALN OR: 7.3 (2.4-25) P=.00. Conclusion: This preliminary case control study showed an increased risk of pre-cancerous colprectal lesions in patients with acromegaly, supporting the VCC screening at diagnosis.

Introdução: o câncer colorretal (CCR) é um problema mundial, com uma incidência anual de cerca de um milhão de casos e uma mortalidade anual de mais de 500 mil. O rastreamento leva a um diagnóstico precoce, sendo uma forma eficaz de diminuir a mortalidade. A pesquisa de sangue oculto nas fezes (PSOF) consiste na identificação de hemoglobina nas fezes e tem sido usada como rastreamento desta neoplasia. Objetivo: o objetivo do presente estudo é verificar, a partir da positividade do exame de PSOF, a prevalência de alterações em exames de colonoscopia. Ainda, objetiva-se avaliar a sensibilidade do método de PSOF para rastreamento do câncer colorretal em um município do meio-oeste de Santa Catarina. Metodologia: trata-se de um estudo transversal descritivo e exploratório de base populacional. Utilizou-se uma amostra populacional constituída por indivíduos com idade mínima de 50 anos que procuraram a Estratégia Saúde da Família (ESF) do município para consulta médica no período entre agosto de 2014 e maio de 2015. Para o rastreamento do CCR considerou-se como método de escolha a PSOF, realizada por método de guáiaco e/ou imuno-histoquímico. Resultados: a amostra foi constituída por 438 indivíduos, dos quais 90,87% (n=398) tiveram seu exame de PSOF negativo. Dentre aqueles com exame positivo (n=40; 9,13%), 55% (n=22) eram do sexo feminino. Em ambos os grupos predominaram os indivíduos cuja cor da pele autorreferida era branca, não tabagista, que não faziam uso de AINES ou de AAS, que não tinham diagnóstico de anemia no último hemograma realizado e cuja história pregressa de neoplasia e doença inflamatória intestinal, bem como a história familiar de pólipo intestinal ou câncer colorretal eram negativas. Dentre os principais achados naqueles que realizaram o exame de colonoscopia por apresentarem a PSOF positiva, verificou-se uma maior prevalência de adenomas tubulares com displasia e de pólipos hiperplásicos, com 22,5% (n=9) cada. Os adenocarcinomas foram observados em 3 indivíduos, correspondendo a 7,5% daqueles com PSOF positivo. Conclusões: os autores concluem que o investimento em diagnóstico precoce e no rastreamento de CCR na população considerada de risco para a doença é fundamental, uma vez que representa um importante problema de saúde pública em Santa Catarina, dada a tendência de aumento na mortalidade.


Introduction: colorectal cancer (CRC) is a worldwide problem, with an annual incidence of about one million cases and an annual death of more than 500,000. Tracking leads to an early diagnosis and an effective way to reduce mortality. The occult blood in the stool (FOBT) is the hemoglobin identification in feces and has been used as a screening of this cancer. Objectives: the objective of this study is to verify, from the positivity of the test FOBT, the prevalence of changes in colonoscopy exams. Still, the objective is to evaluate the sensitivity of FOBT me-thod for colorectal cancer screening in a city in the Midwest of Santa Catarina. Methodology: this is a descriptive and exploratory cross-sectional population-based study. We used a population sample of persons aged 50 years searching the Family Health Strategy (FHS) in the city for medical consultation from August 2014 to May 2015. To trace the CCR was considered as method of choice FOBT performed by method of guaiac and / or immunohistochemistry. Results: the sample was constituted by 438 individuals, of which 90.87% (n=398) had their test negative FOBT. Among those with positive test (n=40; 9.13%), 55% (n=22) were female. In both groups predominated individuals whose self-reported Skin color was white, non-smokers who did not use NSAIDs or aspirin who had no diagnosis of anemia in the last blood count done and whose previous history of cancer and inflammatory bowel disease as well such as family history of intestinal polyps or colorectal cancer were negative. Among the main findings in those who underwent colonoscopy because they have a positive FOBT, there was a higher prevalence of tubular adenomas with dysplasia and hyperplastic polyps, 22.5% (n=9) each. Adenocarcinomas were observed in 3 subjects, corresponding to 7.5% of those with a positive FOBT. Conclusions: the authors conclude that investment in early diagnosis and CCR tracking the population considered at risk for the disease is crucial, since it represents an importante public health problem in Santa Catarina, given the trend of increased mortality.

ABSTRACT

O câncer colorretal (CCR) é atualmente considerado uma neoplasia com alta taxa de incidência e de mortalidade na população, sendo o terceiro tipo de câncer mais prevalente no mundo e a segunda causa de mortes por câncer. Devido à importância de amenizar estas taxas, foram avaliados diversos métodos existentes nos Estados Unidos, na Europa e no Brasil para o rastreio do câncer colorretal, sendo notável a variedade de opções de exames disponíveis, além dos fatores de risco mundialmente difundidos. Foi observado que a mudança do estilo de vida é de difícil adesão, tornando assim imprescindível a adoção de um método de rastreio eficaz para detecção precoce e remoção de lesões pré-malignas e de neoplasias em estágio inicial, visando reduzir a incidência e a mortalidade do CCR e os gastos despendidos com o tratamento no caso de uma neoplasia avançada. No entanto, ainda não há um consenso acerca de um método de rastreio altamente qualificado e utilizado mundialmente de maneira unânime (AU)


Colorectal cancer (CRC) is currently considered a neoplasm with a high rate of incidence and mortality in the population, being the third most prevalent type of cancer in the world and the second cause of cancer deaths. Due to the importance of mitigating these rates, several methods for the screening of colorectal cancer used in the United States, Europe and Brazil were assessed, being noteworthy the variety of options for tests available, in addition to the risk factors broadcast worldwide. It has been observed that lifestyle changes are difficult to implement, thus making it imperative to adopt an effective screening method for the early detection and removal of premalignant lesions and early stage neoplasms in order to reduce the incidence and mortality of CRC and the costs incurred with treatment in case of advanced neoplasia. However, there is as yet no consensus on a highly qualified screening method used universally worldwide (AU)

OBJETIVO: Determinar la relación que existe entre el consumo de la dieta vegetariana y el cáncer colorrectal en adultos de dos hospitales de Lima. MATERIAL Y MÉTODOS: Se realizó un estudio analítico de casos y controles considerando exposición al tipo de dieta y efecto al cáncer colorrectal. Se incluyeron 142 casos y 143 controles. Los casos fueron adultos con cáncer colorrectal diagnosticados mediante biopsia y el control fue el adulto sin cáncer colorrectal, atendidos en el mismo período y hospital. Se aplicó una encuesta previamente revisada por expertos, se efectuó un estudio piloto para evaluar comprensión y se realizaron los ajustes pertinentes. Se calculó chi cuadrado, OR y regresión logística. RESULTADOS: La mediana de edad fue 62 años, predominantemente mujeres, la mayoría con nivel educativo secundario. El análisis bivariado mostró que el sexo (X2=13.01; valor p<0.01), el nivel educativo (X2=11.31; valor p=0.01) y el antecedente familiar de cáncer (X2=5.07; valor p=0.02) estuvieron asociados al cáncer colorrectal; mientras que el consumo de AINES (X2=0.009; valor p=0.924), tipo de dieta ((X2=0.60; valor p=0.44)) y la actividad física (X2=0.46; valor p=0.50) no tuvieron asociación significativa. La regresión logística demostró que la dieta vegetariana (OR=0.05; IC 95%: 0.01; 0.59) y la edad (OR=0.94; IC 95%: 0.89; 0.99) estuvieron asociadas al cáncer colorrectal. CONCLUSIONES: El consumo de la dieta vegetariana sería un factor protector del cáncer colorrectal en la población estudiada.


OBJECTIVE: To determine the relationship between the consumption of a vegetarian diet and colorectal cancer in adults at two hospitals in Lima. MATERIAL AND METHODS: Am analytic case-control study, considering exposition as type of diet and colorectal cancer as the effect, was conducted in Lima, including 142 cases and 143 controls. The cases were adults with colorectal cancer, diagnosed by biopsy, while the control group consisted of adults without colorectal cancer treated at the same hospital, at the same time. Participants responded an instrument that was previously reviewed through a pilot study to evaluate comprehension of the items, and then adjustments were made, with the support of experts. Chisquared was calculated in a bivariate analysis, and a logistic regression model was constructed. RESULTS: The median age was 62 years. The participants were predominantly women, and most had a high school education. The bivariate analysis showed that sex (X2 = 13.01; p value <0.01), educational level (X2 = 11.31; p-value = 0.01), and family history of cancer (X2 = 5.07; p-value = 0.02) were associated with Colorectal cancer; while consumption of NSAIDs (X2 = 0.009; p-value = 0.924), type of diet ((X2 = 0.60; p-value = 0.44)) and physical activity (X2= 0.46; p-value = 0.50) had no significant association. Logistic regression showed that the vegetarian diet was associated with colorectal cancer (OR = 0.05, 95% CI: 0.01, 0.59) and with age (OR = 0.94, 95% CI: 0.89; 0.99). CONCLUSIONS: Vegetarian diet is a protective factor of colorectal cancer in the study population.
